It's a worrying development that has the Pan American Health Organization (PAHO), a key United Nations agency, sounding the alarm: measles, a disease once thought to be largely under control in the Americas, is making a significant comeback. In an urgent epidemiological alert issued on Wednesday, PAHO has implored its member states to bolster their routine surveillance and vaccination efforts to get a handle on this escalating public health concern.
But here's where it gets concerning: PAHO's data paints a stark picture. In just the first three weeks of 2026, a staggering 1,031 cases of measles were documented across the region. This follows a total of 14,891 confirmed cases throughout 2025. These aren't isolated incidents; PAHO has pinpointed North America as a particular hotspot, with countries like the United States, Mexico, and Canada grappling with substantial outbreaks.

Measles is an incredibly contagious airborne virus. Imagine this: if you're unvaccinated and come into contact with someone who has measles, there's a 9 out of 10 chance you'll catch it. While most people recover within a few weeks, the disease can have serious, even life-altering, consequences, especially for the youngest among us. We're talking about potential complications like ear infections, lung inflammation, pneumonia, and even encephalitis – a dangerous swelling of the brain that can lead to permanent damage, including seizures and memory loss.
The MMR vaccine (measles, mumps, and rubella) is our most powerful weapon against this virus, offering robust protection. The standard recommendation for healthy children is to receive their first MMR dose before 15 months of age, with a second, final dose recommended before they turn six. Despite the clear benefits, vaccination rates have unfortunately been declining in some countries, including the United States. This decline is often fueled by misinformation and conspiracy theories. For instance, there have been claims that the MMR vaccine's protection wanes quickly or that it carries significant health risks. However, medical experts, including those at the US Centers for Disease Control and Prevention (CDC), consistently affirm that the vaccine provides lifelong protection and is overwhelmingly safe. The Infectious Diseases Society of America has even stated that "there have been no deaths shown to be related to the MMR vaccine in healthy people."
A Closer Look at North America's Challenges
Let's dive into some of the specific numbers. In the first three weeks of 2026, the US reported 171 new measles cases, adding to the 2,242 cases seen in 2025. One particularly concerning outbreak in South Carolina has seen 876 reported incidents, with the vast majority of those affected being unvaccinated. Similarly, a significant outbreak in Texas between January and August 2025 resulted in 762 cases, tragically leading to the deaths of two unvaccinated children and 99 hospitalizations.
It's important to remember that measles was declared eliminated from the US in 2000, and the entire Americas region was declared measles-free in 2016. However, with these rising case numbers, both the US and Mexico are at risk of losing their elimination status. Mexico, in particular, reported a concerning 6,428 cases in 2025, the highest in the Americas, with an additional 740 cases in the first three weeks of 2026.
PAHO is scheduled to review the measles elimination status of the US and Mexico in a virtual meeting on April 13. Meanwhile, Canada has already had its measles elimination status rescinded in November, following several outbreaks since October 2024. Canada saw 5,436 cases last year and 67 in the first three weeks of 2026. To regain its status, Canada must demonstrate the cessation of measles transmissions from its outbreaks for over a year.
